Forming a health system which is capable of providing high-quality, effective care at the lowest possible cost requires getting value-based payment and delivery models working. To achieve this, all members of a patient’s care team including the patients and their caregivers (e.g. family members) must be engaged. They have to collaborate in an atmosphere of trust. For patients this means that they need to be empowered to stay well and to assume more responsibility for their health. For providers this requires to shift focus towards patient-centered care to achieve a tighter involvement with their patients.

Major contributions to meeting these challenges will be obtained from enabling patients and their caregivers to access health information, to frequently communicate with providers and to share a common view on upcoming activities. ICW’s Patient Engagement solution embraces these challenges by giving access to the patient’s longitudinal record, by enabling data-driven electronic conversations, and by allowing bilateral assignment of health care activities. Patients are notified on new data coming in from connected providers, allowing them to track the progress on their individual care plan and to increase data quality by double checking data consistency.

EHR Modular Compliance Certificate (Ambulatory) ICW Patient Portal 1.2

ICW Patient Portal 1.2 is a certified EHR Module and works together with your certified EHR software to help you meet Meaningful Use (MU) attestation. ICW Patient Portal 1.2 is certified for the following Stage 1 and Stage 2 measures:

• §170.314(e)(1) View, Download, Transmit
• §170.314(e)(3) Secure Messaging

Cost and Limitations

This certified product-version will generally require one-time implementation costs for set-up, configuration, customization and integration of the modular EHR system. The implementation services will be offered on a time and material basis and the resulting fees are related to the number of interfaces and the complexity of the existing infrastructure. Reporting on the automated measure calculation requires a license for a compatible reporting tool like e.g. Microsoft Excel. This license is usually a one-time software license.

In addition, this certified product version will require either a one-time software license and annual maintenance fee or an ongoing subscription fee for use of the solution including maintenance, upgrades and support. The fee for the ICW Patient Portal is based on the number of patients that are registered within the system. These costs are usually covered by the organization operating the patient portal (e.g. an independent practice association or an integrated delivery network) not by the provider facility.

Both of these costs will be in addition to any costs associated with the operation of the infrastructure as well as other required certified EHR modules.

There are no limitations with respect to the usage scenario, transactions nor associated bandwidth.
